.634 -> - [Dr. Devore] So CONNECT-HF is a quality improvement trial.
.319 -> We're trying to test different strategies, to try to improve
.731 -> the care of patients who are hospitalized
.553 -> with acute heart failure.
.642 -> In the United States right now there're really a hyperfocus
.837 -> on trying to reduce 30-day readmissions,
.609 -> and CONNECT-HF has really focused on more of the care
.319 -> that the patients receive for the next year
.15 -> after they're in the hospital.
.613 -> We're enrolling patients now.
.759 -> we've enrolled a little over 700 patients
.54 -> and we're on our way.
.716 -> It's a really exciting time and we're really pleased
.763 -> with how the study's gone so far.
.162 -> So one of the things is we're meeting with hospitals
.547 -> around the country and learning what they're doing
.976 -> to try and reduce readmission rates
.442 -> and improve the care of patients at their hospital.
.763 -> So some of those hospitals are being randomized
.983 -> to an intervention where we're capturing their data,
.141 -> providing a lot of feedback, and then remeasuring
.882 -> to see how they're doing with the care they're providing.
.198 -> That's really the biggest intervention,
.62 -> and where we hope to make an impact here.
.023 -> We also have some really important sub-studies, though.
.094 -> One is around using different apps
.644 -> to try to engage patients with some of their behaviors.
.535 -> So some of it is around activity,
.734 -> but a lot of it is around medication adherence,
.194 -> where we know we can have a really big impact
.577 -> on patient outcomes.
.367 -> It's really exciting; we're learning a lot
.668 -> about the use of apps in these patients,
.369 -> and also different behavioral-economic principles
.273 -> that might be helpful to help patients
.458 -> care for heart failure.
.595 -> I became interested in heart failure
.187 -> as I went through my training.
.645 -> I always really liked having a really great relationship
.804 -> with patients, and so I really liked the idea of
.722 -> being able to care for patients over a long period of time.
.991 -> Heart failure is a chronic disorder where you really get
.712 -> to know your patients and help them
.938 -> through the thick and thin.
.658 -> And then I always loved the physiology.
.487 -> Heart failure is really fascinating,
.799 -> and it's a really exciting time to actually be
.021 -> a heart-failure cardiologist,
.121 -> because we have a lot of new therapies,
.785 -> but we don't know how to implement them.
.403 -> And so that's where I spend a lot of my efforts.
