Understanding the causes of Heart Failure | Dr. Siddhartha Mani (Bengali)

Heart Failure is a condition in which the heart muscle doesn’t pump blood as well as it should. It could occur due to narrowed or clogged arteries (Coronary Artery Disease) or even high blood pressure. The most common symptoms include shortness of breath, fatigue, swollen legs and rapid heartbeat. However, this is a treatable condition.

Dr. Siddhartha Mani is a Consultant Interventional Cardiologist at RN Tagore Hospital, Kolkata, explains the Causes of Heart Failure. According to the doctor, the patient, If diagnosed with Hypertension or clogged arteries, should reduce salt intake, aim for a healthy lifestyle, and take the prescribed medication. In some cases, a defibrillator or pacemaker may also be implanted.

14.895 -> Hello. I am Dr Siddhartha Mani,
17.477 -> a consultant, interventional cardiologist
19.73 -> at R N Tagore Hospital in Mukundapur (Kolkata),
22.752 -> Today, I'll talk about heart failure,
26.508 -> What does heart failure mean.
30.565 -> The main work of the heart is to pump blood.
34.593 -> The heart continuously contracts and relaxes to cause the blood to flow in the arteries and the veins in the body.
43.511 -> When this pumping capability is obstructed for any reason,
47.905 -> means when it pumps slowly,
52.199 -> or it does not relax properly or does not get filled properly.
57.846 -> then, the blood flow in various organs in the body becomes less.
62.976 -> If the heart does not pump blood, it won't reach the arteries and the veins.
66.888 -> Thus, it won't reach the organs.
69.531 -> The blood will get stored in the lungs as water or fluid due to back pressure.
75.801 -> Then, the oxygen exchange in the lungs gets disrupted.
81.011 -> The patient will have breathing problem, and the oxygen saturation will be less.
84.085 -> All these occurrences are altogether called heart failure.
91.491 -> There are various reasons behind heart failure.
96.529 -> But, this can be categorised as a disease of muscles.
100.615 -> This means that the muscles fail to contract and relax properly.
105.175 -> There are many reasons for that. It may be because of hypertension
108.732 -> or blood sugar or an ischemic heart,
114.049 -> The muscles does not get enough blood as there is blockage in the coronary artery
118.839 -> Or, a part of the heart is not moving properly due to an heart attack.
123.345 -> In such cases, the heart cannot pump properly.
127.599 -> There are times when the heart is unable to relax due to hypertension.
132.804 -> Both contraction and relaxation are very important.
135.2 -> So, diastolic heart failure may also be a cause for breathing problems and heart failure.
140.933 -> What are the symptoms of heart failure?
142.841 -> Breathing problem, swelling in feet.
146.324 -> At times, water storage in the lungs
150.222 -> These are the main symptoms
152.44 -> In addition, we correlate with other diseases to come to the diagnosis.
159.906 -> What is the treatment for heart failure?
162.682 -> Firstly, the primary disease needs to be addressed.
165.679 -> If there is sugar, pressure or a ischemic heart disease, it needs to be treated.
169.615 -> If there is a block in the coronary artery, it has to be opened through angioplasty.
173.378 -> Or, if the muscle has weakened due to some myocarditis viral or bacterial infection,
180.882 -> that needs to be treated.
183.113 -> Firstly, the primary disease must be treated
186.611 -> Secondly, taking medicines to increase the heart's medicine and stop water storage in the lungs.
195.095 -> There are some rules
196.725 -> One important rule is fluid restriction.
199.662 -> We stipulate the amount of fluid intake for the entire day, a litre or so
207.058 -> That measurement needs to be followed
208.705 -> Except that, the intake of raw salt and oily food is restricted,
213.783 -> In some cases, the condition of the heart is so bad
217.624 -> that they need to use pacemakers.
220.855 -> There are types like AICD or CRTD
223.925 -> These increase the pumping and lessen the complications of a sudden cardiac death
228.786 -> Such pacemakers help us control heart failure to a large extent.
234.268 -> Such pacemakers can be used
236.215 -> other than this, when it is an extreme case, and nothing seems feasible.
239.357 -> The pumping has drastically decreased
241.952 -> What is normally 65-70 percent has now decreased to 20-25 percent
247.968 -> And, we have no other way.
250.217 -> Then, we have to think of a heart transplant
252.285 -> Heart transplant is more easily available these days even in our state
259.084 -> It has already started in our hospital as well
262.311 -> We are doing it on a regular basis
265.65 -> We may have to think of that treatment
267.046 -> Heart failure comes with minimal symptoms at times.
270.67 -> Take note if there is swelling in leg or fatigue, or you get out of breath while walking.
276.096 -> Do visit a doctor.
277.34 -> Get the tests done and get diagnosed.
279.054 -> And, get treated. Thank you.
